Continuing Care Network

The Continuing Care Network (CCN) is a partnership that includes Virginia Mason Franciscan Health resources, as well as select local care facilities, providers and agencies. Our goal is to provide the best possible options for our patients who need continuing care after they leave the hospital.

Choosing the right setting for your loved one is important, and it can be challenging. Our CCN partners share our strong desire to ensure a smooth transition, effective communication and continued quality care after an acute hospital stay.

The Continuing Care Network supports patients in the following ways:

• Providing oversight for patients with complex medical needs
• Overseeing the quality of care and patient satisfaction in our CCN
• Striving to improve the recovery experience for patients and their families, and promote optimal return to function while reducing unnecessary readmissions to the hospital

Partners in our network have undergone rigorous site review and many meet or exceed federal quality standards and state patient satisfaction ratings. Rainier Health Network oversees the quality of care and patients satisfaction in our CCN.
